Smile CDR is built on HAPI FHIR and adds enterprise modules (terminology, interoperability, MDM), tooling, and commercial support. Running HAPI directly is free and fully under your control, but you build and maintain everything yourself. Smile makes sense when the enterprise capabilities and support are worth the license; direct HAPI when you have the engineering depth and want no licensing.
Self-hosted vs managed Smile?
