What Mirth Connect Services Cover

Mirth Connect Licensing After March 2025

Mirth Connect licensing changed significantly in March 2025, when NextGen Healthcare moved new releases to a commercial licence. Organizations running Mirth Connect now face a real decision: stay on the last open-source version, move to a commercial licence, or migrate to an open-source alternative. Each option has different costs, security implications and support paths, and the right choice depends on how many channels you run and how critical they are. What Changed in March 2025

With version 4.6, NextGen Healthcare moved Mirth Connect to a commercial licence, ending free access to new releases. Existing open-source installations keep working, but they no longer receive new features or security updates from NextGen without a paid commercial licence in place.

Staying on the Last Open-Source Version

Remaining on version 4.5.2 avoids licence cost in the short term. The trade-off is that security fixes stop arriving, so risk grows over time. This option suits stable, low-change environments with a planned exit date and strong network controls around the engine.

Mirth Connect Integration With EHRs and Billing Systems

Most Mirth Connect work exists to connect an EHR to something else: a lab system, a billing platform, a patient app, a registry or another EHR. Each EHR vendor exposes data differently, through HL7 v2 feeds, FHIR APIs, flat files or proprietary web services, and each has its own testing process and constraints. Support retainers typically cover 20 to 80 hours per month for monitoring, incident response, patching and small changes. The right size depends on channel count, message volume and response time requirements, which we agree before the retainer starts. Retainer size can change each quarter.
